Individual and Transitional Support is a direct, one-on-one service provided to a beneficiary age 16 and older with a Serious Emotional Disturbance (SED), Serious Mental Illness (SMI) or Severe and Persistent Mental Illness (SPMI). This service provides structured, scheduled interventions to improve a beneficiary’s ability to manage instrumental activities of daily living (IADL) and promote independent functioning in the community.
For adults with SMI or SPMI, the intent of the service is to teach and assist a beneficiary in carrying out IADLs, such as preparing meals, managing medicines, grocery shopping and managing money, so they can live independently in the community.
For a beneficiary age 16 to 20 with SED transitioning to adulthood with at least one deficit in an IADL, this service provides support in acquiring, retaining and improving self-help, socialization and adaptive skills necessary to be successful in employment, housing, education and community life and to reside successfully in the community. Activities are provided in partnership with youth to help the youth arrange for the services they need to become employed, access transportation, housing and continuing education
