In this program, individuals will learn ways to develop a healthy balance in life, be given guidance in decision making, offered support in navigating mental health difficulties, and assisted in establishing a recovery plan. Coaches and peer support specialists help individuals to find ways of abstaining and maintaining stability, managing difficult symptoms, rebuilding relationships, finding purpose for living by creating a clear plan of action, and using current strengths to reach future goals.

During this time testimonies are shared of how one woman made it out of the lion's mouth and came back to the wilderness stronger than how she left. She will show others how she is no longer a victim but victorious.

The Five-Step Pathway to success while on your journey to a healthy mental health
"You are on your way to recovery"
Step 1
Talk to Us: 1-on-1 Meeting Intake Process:
In order to take the person towards his or her destination of recovery he or she must first tell the coach or peer support specialist where he or she would like to go. In this process an intake will be explained and completed by asking questions and filling out consent forms. The assisgned intake coordinator will ask if the person has any questions that pertains to the program. After all questions has been answered the coordinator, mental health coach or peer suppoort specalist will sign the intake form. During this time a handbook will be explained and provided to the person seeking services. An orentation form will be given to the person and explained throughly. After the orentation form has been reviewed the person seeking services will place his or her intials in the area where information of the program was explained. The next step: An assessment will be schuduled with a licensed clincial mental health therapist or license clinical addictions specialist counselor for individiuals with a subatance use diagnose. Step #2 will begin!
Step 2
Assessment: Clincial Comprehensive Assessment known as (CCA).
In order to provide the person with services to fit his or her needs the individual seeking services will have to complete an asessment. An assessment is what gets the service started and helps to tell what services may be needed and recommended by the clincial mental health therapist completing the assessment. Look at it this way getting a completion of an assessment is like getting a xray done. After the reading of an xray the doctor can now treat the person of the problem that has caused the agony and will inform the patient the best route to take after the findings of reading the xray.
The assessment is the done simliar . For example, after the person informs the therapsit what he or she has experince that's causing the distress in his or her life whether it is from mental health and/or substance use and anyother events that has taken place in the person's life the therapist gathers all the information and recommends what service or services that best fit the needs of the person. After the completion of the assessment step 3 will take place.
Step 3
Person Centered Plan knows as (PCP)
The PCP will be developed with the person served and it would be built upon the information from the assessment and the stated goals of the person seeking services. Think about it this way; a PCP is a living will for treatment and it can be changed upon request at anytime. Each time the mental health coach and/or peer support specialist meet with the person served he or she will work on the goal/s that has been established and agreed upon by the person receiving services. Each meeting will reflect the decision of the person served of what goal/s he or she would like to work on that day. The creation of the PCP will not happen without the individual. The PCP is exactly what it stands for it is centered around the person receiving services. After the completion of the PCP now it's time to move trowads step 4.
Step 4
The Program
After the PCP has been created signed by the Qualified professional and person seeking services the agency will send all required documents to the person's insurance and/ or MCO to determine medical necessity. Medical necessity is based on evidence based clinical standards of care. This simply means that there is evidence to support a course of treatment based on a set of symptoms or other diagnostic results. Once approved now the work begins towards creating a healthy journey towards recovery. Now the work begin and the assigned staff with assist the person served to create a wrap plan which is step 5.
Step 5
WRAP IT UP!
After all documents are submitted and approved you will get assigned to a personal mental health coach/ peer support specialist. The coach will work closey with the person served in creating a wrap plan. A wrap plan is a document that monitors wellness, times of being less well and times when experiences are uncomfortable and distressing. Look at it this way wrap plans are documents that bundle health and welfare into a single plan. It is the prescription to a better health and wellness which helps creates a better you. WRAP stands for : Wellness Recovery Action Plan. Our job is to help the person served to create a healthy plan so he or she will start the journey towards living a healthy life.
